You can reproduce this bug by creating a missile guidance that only accelerates during boost so that it is obvious when the missile is in boost phase, then putting this guidance on a missile that uses a propellant-oxidizer combo.
The missile completely ignores whatever % boost setting you put on it and continues boosting forever, running out of delta-V and self-destructing long before it ever gets near the target if using a high-thrust missile.
In my case my missile was using LOX-methane rockets.
I can confirm that the missile is stuck in boost phase because if I manually force midcourse or terminal, the missile stops thrusting.
I suspect that the missiles are not able to tell when they have enough delta-V to enter terminal phase either but I am not sure how to test it yet.

I think I may have gotten something wrong here since I just created a single-propellant missile (nitromethane) and it also has this problem...

Also to be clear, this issue occurs only with missiles launched during ship-to-ship combat.

Yep! I came back to post that I further found out that the issue seems to be caused by missiles with excessive acceleration (my missiles were capable of 20Gs, compared to the stock striker's 5Gs), which seems to be what is described there.
